VI. If needed, I have an active license and we will be doing medical certification online.

VII. State will collect a $40 payment, then forward your application to me (only including your name and DOB).

VIII. Important that you send me an email at scottjay.doctor@gmail.com, containing your name and phone number (email address will be linked), the state does not forward it to me.

IX. I reach out to you to schedule a video call (FaceTime and Google Duo are most common). Takes about 15 minutes.

X. During our video call, once I complete the state medical certification, you will get an important email with the heading noreply@accela.com which will work at stores for you from the moment it arrives (email good for 30 days)

XI. Your card comes by mail in about one week.

XII. My fee of $100 is collected at the end or after the video call. The total cost to you is $140, split $40 to the State first and $100 to Doctor last. Card good for 2 years.

XIII. State helpline (in Lansing, 5 days a week) 517-284-6400
